Client: Northern Cochise Community Hospital / Wilcox, AZ
Project:
Provide overall management and administration of this hospital based skilled care facility.
Placement of a licensed nursing home administrator on site
Goal:
Insure the overall operation of the skilled nursing facility provides the highest level of resident care while insuring compliance with all federal and state rules and regulations.
Maintain the overall financial stability and viability of the skilled nursing facility within today’s financial environment.
Client: AK Chin Indian Community - American Indian Health Management and Policy / Maricopa, AZ
Project:
Conduct a comprehensive tribal and community wide medical and health assessment.
Produce an equally comprehensive report detailing the opportunities for self determination in the provision of health and medical services with the potential of developing a tribal medical center.
Goal:
The improvement of health and medical services for Ak Chin Indian Community members through improvements to access, choice, quality and delivery.
Client: The Confederated Tribes of the Umatilla Indian Reservation (CTUIR) / Pendleton, OR
Project:
To analyze the current array of tribal elder services, the departments and programs that deliver those services including the consumer, the tribal elder. To recommend a more efficient and more effective system of service deliver for elder services.
Goal:
To recommend and restructure a more effectively organized tribal elder care delivery system (department, program, and service) with more positively received services and satisfied elders.
Client: Colorado River Indian Tribes (CRIT) / Parker, AZ
Project:
Submission of a proposal to IHS for a two year planning grant to assist CRIT in the design and development of an elder service delivery system. The grant was funded.
Goal:
The establishment of the CRIT Elder Care Committee, completion of tribal feasibility studies covering assisted living and skilled nursing care. Planning for year two funding underway.
Client: Tohono O’odham Nursing Care Authority (TONCA) / Sells, AZ
Project:
Multi year agreement to provide management services for the
Archie Hendricks Sr Skilled Nursing Facility and Tohono O’odham
Hospice. Planning and development of assisted living housing.
Goal:
Facility is fully staffed and occupied. Facility is well received by tribal
members. Successful Medicare certification. Numerous awards including Highest Honors, Harvard Project on Native American Self Determination.
Client: Nez Perce Tribe / Lapwai, ID
Project:
Meet with tribal staff and elders regarding approaches to elder services and present findings at the Elders Conference on elder care services, programs and how to develop tribal elder services.
Goal:
Assist the tribal health care providers to inform, educate and plan with elders. The result is more informed elders who are prepared to work with staff on elder care planning.
